Description: A clinical performs one or more of the following services to provide information or materials for use in the diagnosis, prevention, or treatment of a disease or the identification or assessment of a medical or physical condition. Services include examination of fluids, tissue, cells, laboratory or other materials taken from the human body. All facilities, including physician offices, performing non-waived clinical laboratory testing, are required to get a federal Clinical Laboratory Improvement Amendment (CLIA) certificate and a state clinical laboratory license. Providers performing only waived testing who do not use a microscope are required to get a federal CLIA Certificate of Wavier. With the exception of providers doing only provider performed microscopy procedures, survey requirements apply only to those laboratories performing non-waived testing.CLIA waived laboratories are not found in the Facility/Provider locator tool.
